Parking Permits (Permanent or Temporary)
- Fill out Part 1 of form & sign with your full name; Part 2 must be completed by your doctor’s office.
- Provide form to Town Clerk’s Office, 1350 Turk Hill Road, Fairport, NY 14450.
- Include a copy of your driver’s license or non-driver’s ID.
- Temporary permits are good for up to six months; permanent permits must be updated every three years.
