Perinton Town Clerk Janelle Reed is pleased to announce the return of Family Passport Day, taking place on Saturday, May 3, 2025, from 9 a.m. to noon at the Perinton Town Clerk’s Office in Perinton Town Hall, 1350 Turk Hill Rd., 14450. This special event provides a hassle-free, appointment-only opportunity for Perinton residents to apply for passports—perfect for busy families preparing for their next adventure.

By Appointment Only
To ensure an efficient process, passport services will be available by appointment only. Residents can begin scheduling their appointments on Wednesday, April 9, 2025, by calling the Town Clerk’s Office at (585) 223-0770. This appointment-based system ensures personalized service for each applicant, making the process as smooth as possible.
Passport Services at Perinton Town Clerk’s Office
The Perinton Town Clerk’s Office is a designated Passport Acceptance Facility by the U.S. Department of State, offering services for first-time adult applicants and minors. Adult passport renewals using form DS-82 do not need to go through an Acceptance Facility if they are less than five years expired and can be mailed directly to the U.S. Department of State by the applicant.
Passport Types
- U.S. Passport Book – Valid for international travel by air, land, or sea.
- U.S. Passport Card – Valid for international travel by land or sea to Canada, Mexico, Bermuda, and the Caribbean (not valid for air travel).
Application Requirements
For Adults (Age 16 & Older):
- Applicants must appear in person.
- Applicants ages 16 and 17 must be accompanied by at least one parent or legal guardian.
- Completed DS-11 passport application (Black ink only, NO white-out, NOT SIGNED).
- One 2”x2” passport photo (available at the Town Clerk’s Office for $10).
- Birth Certificate with raised seal and both parents’ full names listed.
- Government-issued proof of identification (passport, driver’s license, learner’s permit, etc.).
- Previous passport (if applicable).
- Social Security number must be included on the application.
For Minors (Age 15 & Younger):
- Child must appear in person.
- Both parents or legal guardians must be present.
- Completed DS-11 passport application (Black ink only, NO white-out, NOT SIGNED).
- One 2”x2” passport photo (available at the Town Clerk’s Office for $10).
- Birth Certificate with raised seal & both parents’ full names listed.
- Both parents must provide government-issued identification.
- Student ID for the applicant, if available.
- Social Security number must be included on the application.
Fees & Payment Information
The following fees must be paid via check or money order made out to the U.S. Department of State:
- Passport Book – Adult: $130.00
- Passport Book – Minor: $100.00
- Passport Card – Adult: $30.00
- Passport Card – Minor: $15.00
- Expediting Fee (Adult or Minor): $60.00
The following fees can be paid via cash, check, or credit card (with applicable fee) to the Town of Perinton:
- Passport Photos: $10.00
- Acceptance Fee: $35.00
