The Perinton Town Clerk’s Office is thrilled to announce Family Passport Day, taking place on Saturday, November 4th, from 9 a.m. to 12 p.m. This event is for Perinton residents and provides a convenient opportunity for busy families to get their passport applications completed.

By Appointment Only:
To ensure a smooth and efficient process, we are offering passport services by appointment only. Beginning on October 13th, you can schedule your appointment by phone. This appointment-based system allows us to provide personalized information to each family, making the passport application process hassle-free.
Passport Services at Perinton Town Clerk’s Office:
The Perinton Town Clerk’s Office is a designated Passport Acceptance Facility by the US Department of State and offers this service to adults applying for a passport for the first time and to minors. Renewals for adult passports using form DS-82 are not required to pass through an Acceptance Facility, if they are less than 5 years expired, and can be forwarded by mail directly by the applicant to the US Department of State.
Passport Types:
The US Passport Book can be used for international travel regardless of the form of transportation or destination.
The US Passport Card can be used only for international travel to Canada, Mexico, Bermuda, and the Caribbean by land or sea only. The Passport card is NOT an accepted form of international identification when traveling internationally by air or to destinations other than those listed above.
Requirements:
ADULT PASSPORT BOOK OR CARD – APPLICANTS AGE 16 & OLDER:
- Applicants must appear in person.
- Applicants ages 16 and 17 must be accompanied by at least one parent or legal guardian.
- Completed DS-11 passport application form – Black ink only, No white-out, NOT SIGNED.
- One 2”x 2” picture with a white background (can be obtained at the Town).
- Birth Certificate with a raised seal and both parents’ full names listed.
- If not born in the United States, another official form of proof of United States citizenship.
- Government-issued proof of identification such as a passport, driver’s license, learner’s permit, etc.
- Previous Passport if applicable.
- The social security number must be included with the application where indicated.
MINOR PASSPORT BOOK OR CARD – APPLICANTS AGE 15 & YOUNGER:
- Minor must appear in person.
- BOTH parents or legal guardians must be present.
- Completed DS-11 passport application form – Black ink only, NO white-out, NOT SIGNED.
- One 2”x2” picture with a white background (can be obtained at the Town).
- Birth Certificate with a raised seal & both parents’ full names listed.
- If not born in the United States, another official form of proof of United States citizenship.
- Both parents must provide government-issued proof of identification such as a Passport, Driver’s License, learner’s permit, etc.
- Student ID for the applicant, if available. The social security number must be included in the application where indicated.
PASSPORT-RELATED FEES:
PASSPORT BOOK & CARD FEES:
The following fees must be paid in the form of CHECK OR MONEY ORDER to the US Department of State:
- Passport Book – Adult: $130.00
- Passport Book – Minor: $100.00
- Passport Card – Adult: $30.00
- Passport Card – Minor: $15.00
- Expediting fee (adult or minor): $60.00
PASSPORT PROCESSING & PHOTOS:
The following fees can be paid in the form of CASH, CHECK, or CREDIT CARD (w/applicable fee) to the Town of Perinton:
- Passport Photos – $10.00
- Acceptance Fee – $35.00
